From Narrative to Evidence: How NLP-Extracted Clinical Facts Strengthen Real-World Research

As demand grows for more robust real-world evidence (RWE) to support research, healthcare decision-making, and market access activities, important data gaps remain. While structured electronic health record (EHR) and claims data provide a valuable foundation for real-world research, many clinically meaningful patient characteristics, including disease severity, performance status, biomarker results, treatment response, and functional decline, are often documented only in narrative clinical notes. As a result, cohorts built from structured data alone may not fully represent the patient populations researchers seek to understand. This presentation will explore how clinical facts extracted from unstructured clinical notes can help address these limitations, provide a more complete view of the patient’s journey, and support a broader range of research questions. Attendees will gain insight into the types of clinically relevant information frequently missing from structured data alone and how advances in artificial intelligence (AI) and natural language processing (NLP) are enabling the scalable extraction of clinically relevant facts from narrative documentation. Through practical examples and real-world use cases, we will demonstrate how these enriched data assets can strengthen cohort identification, improve characterization of patient populations, and provide deeper visibility into treatment pathways, disease progression, care patterns, and outcomes. We will also discuss the implications for life sciences researchers, healthcare organizations, payers, and policymakers seeking more comprehensive evidence to support research and healthcare decision-making. Finally, the session will examine the future role of AI-enabled clinical data in advancing evidence generation and supporting data-driven healthcare decision-making.
